Conclusions: Crack-cocaine-related health outcomes are more frequent than previously thought. Crack use is associated with increased odds of lower quality of life, substance use, sexual behavior, physical health, and mental health-related problems. Data on quality of life indicate life expectations are lower for crack-cocaine users than for nonusers and that these differences are more pronounced in black compared with white crack-cocaine users. Longitudinal studies on crack-cocaine users are needed to assess the long-term health impact of crack-cocaine use on different populations. These studies should evaluate the life expectancy and quality of life outcomes of crack-cocaine users in comparison with other drug-user groups and explore whether crack use predicts a more rapid decline in physical and mental health.

Findings: Of the 24 studies considered, most were cross-sectional (n=12); four prospective studies were included as well. Thirteen studies measured health outcomes in crack users and nonusers, ten used crack-cocaine users, and 21 used crack-cocaine with other substances. Pooled prevalences and odds ratios revealed health outcomes associated with crack-cocaine use. Findings from the included studies are presented in Table 1 and are discussed in detail. A number of studies included participants not limited to crack users. Limitations, potential bias, and the poor quality of many studies were addressed in the review. Limitations: The prevalence of many conditions differed between studies; for example, the prevalence of HIV and blood pressure levels were higher in some studies of crack-cocaine users than in others, possibly because of differences in study populations, recruitment methods, and other methodological aspects. Only 11 studies conducted a medication history in crack-cocaine users or nonusers; these 11 studies reported prescription use, often including opioids. Associations between smoking crack-cocaine and smoking more cigarettes per day were identified, suggesting smoking crack-cocaine may increase the risk of tobacco dependence. Other methodological limitations included the use of self-reported, clinical diagnostic, and behavioral questionnaires, with the potential for recall or social-desirability biases; the possibility of selection or information bias; and the inability to control for confounding factors.
